trans people are neurologically different from cis people, the sex center of their brains tend to be more in line with the sex centers of cis people of their experienced gender, and they have characteristic structural features in brain areas related to self-perception and body ownership.

additionally, things like depression, lack of motivation, brain fog, nausea, fatigue, osteoporosis, etc., are actually not associated with hrt. Brain fog can be caused by certain drugs like spironolactone, which is a side effect unrelated to the hormonal aspect of transition. Osteoporosis isn't caused by hrt at all, it's only something that happens if you block hormones and don't also take estrogen. Like how women in menopause get osteoporosis if they don't take estrogen, same mechanism.
