something unusual clearly happened in this case. if you look at the number of incidents this system is processing, it would be surprising if nothing ever got recorded incorrectly.
i think the most likely explanation is that the wrong reason was recorded for the kid's suspension. if it were policy to ever use that reason to suspend a kid, it would have happened more than once. if it had happened to any kid, it's likely we'd be hearing from the kid's parents instead of from """journalists""" digging through official data.
